Mother's Day Care Package
**Notes:
1. All the above prices are quoting in one unit of each item
2. Ghirardelli Chocolate quoted 3 pieces for $1.00
3. The “Tea Bags” is a package of 5 tea bags each.
4. The “Candle” is a package of 4 candles each.
5. Photo frames are options in addition to each package. Please provide a printed photo along before the deadline.
6. All checks should be made payable to Delta Xi Phi Multicultural Sorority Inc.
7. All the order forms should be emailed to: eta_dxp@yahoo.com ; or turned into any of the Delta Xi Phi members
If you have trouble getting the forms, Please email us and we will send you the order forms.
